  • Plan, coordinate, and oversee street outreach activities to engage individuals in need of harm reduction services.
  • Ensure the distribution of harm reduction supplies, such as naloxone kits, syringes, and hygiene products.
  • Develop and maintain relationships with clients, providing direct support and referrals to appropriate services.
  • Supervise and support a team of outreach workers and volunteers, offering training and performance evaluations.
  • Schedule and deploy outreach teams to high-need areas, ensuring effective coverage and impact.
  • Establish rapport with clients through consistent and compassionate interactions.
  • Conduct assessments to identify clients' needs and connect them with relevant resources.
  • Provide crisis intervention and follow-up support to ensure clients' ongoing engagement and progress.
  • Represent Corazon Ministries at community meetings and events to promote outreach efforts.
  • Develop and sustain partnerships with local service providers and organizations to enhance service delivery.
  • Monitor and evaluate the effectiveness of outreach activities, using data to inform program improvements.
  • Prepare regular reports for the Program Director on outreach efforts and challenges.
  • Educate the community about harm reduction principles and advocate for supportive policies.
  • Bilingual in Spanish and English preferred.
  • Bachelor's degree in public health or equivalent experience in a related field.
  • 1+ year of experience in case management and working with individuals experiencing homelessness/substance use/mental health.
  • Proficiency in Microsoft Office Suite and HMIS data management systems.
  • TX Driver's License, reliable transportation, and insurance.
  • Experience in harm reduction practices.
  • Strong understanding of equity and stigma reduction.
