About The Position

The Outreach Coordinator plays a key role in expanding access to behavioral health services by building strong relationships with community partners, referral sources, and priority populations agency-wide. They will play a lead role in the development, implementation, and coordination of Casa’s in-reach and outreach strategies that promote awareness of services as well as support engagement agency-wide. This position increases program visibility, supports timely referrals, improves client access, and helps ensure individuals can navigate entry into care—particularly for individuals facing barriers related to language, stigma, housing, or system complexity. The Outreach Coordinator works collaboratively with clinical, intake, communications, and operations teams to align outreach efforts with program capacity and community needs. As part of the Casa Esperanza, Inc. team, you will work closely with multiple departments and programs to ensure outreach strategies align with organizational goals and that programs are completing their respective outreach objectives. Additionally, you will work with our Marketing and Comms Departments to identify target populations and referral networks to support increasing service utilization.

Requirements

  • Bilingual Spanish/English required.
  • Demonstrated supervisory and project management ability
  • Minimum of four years working in outreach, community engagement, behavioral health, or social services.
  • Experience working with multidisciplinary team models
  • Ability to manage multiple outreach initiatives simultaneously.
  • Experience working with individuals with mental health and/or substance use disorders.
  • Familiarity with social determinants of health and trauma-informed approaches.
  • Ability to work as a team member & independently
  • Strong organizational skills
  • Strong verbal and written communication skills
  • Strong interpersonal skills and crisis management skills
  • Knowledge of local behavioral health systems and community resources.
  • Valid Massachusetts Driver’s License

Responsibilities

  • Work with the Outreach and Marketing team to develop and implement an annual outreach plan that is aligned with organizational priorities, including street outreach to individuals in hotspot locations, community events and presentations, and social media campaigns.
  • Implement in-reach and outreach strategies agency-wide that target our priority populations (e.g., individuals with serious mental illness, substance use disorders, co-occurring conditions, justice-involved individuals, HIV, people experiencing homelessness).
  • Work with the outreach team to develop and maintain a robust referral network that improves client access and referral pathways to treatment, recovery support services, and HIV services, including healthcare providers, shelters, and community-based organizations.
  • Coordinate and facilitate ongoing educational presentations regarding substance use, mental health, HIV, and other health and wellness information to community organizations.
  • Work with the outreach team and marketing department to coordinate campus tours and on-site outreach events.
  • Support Re-Entry Coordinator in developing assertive outreach schedule to correctional facilities, specialty courts, parole/probation boards, and other organizations working with returning citizens
  • Collaborate with the marketing team and direct service staff on the development of outreach collateral materials, patient interviews, and social media campaigns to promote services and engage potential clients.
  • Recruit, train and supervise Promotores to support with outreach, health education, and 1:1 recovery support
  • Coordinate participation in MOAR meetings to connect our recovery community to the statewide addiction recovery community
  • Work with program leadership to understand enrollment needs and service capacity.
  • Represent the agency at community events, health fairs, and stakeholder meetings.
  • Ensure outreach team are trained to educate and support individuals in varying stages of recovery and assists with referrals for food, clothing, shelter, detoxification, and/or medical services
  • Facilitate access to recovery-oriented substance-use-disorder treatment, resources, and recovery groups;
  • Identify barriers to engagement and collaborate with internal teams to improve outreach and referral processes.
  • Support referral coordination and assist community partners in navigating services.
  • Willingly share personal recovery experiences, if applicable, and demonstrate and model effective coping techniques and self-help strategies.
  • Ensure all outreach activities, contacts, and referrals are documented in the agency’s electronic health record
  • Analyze outreach effectiveness and recommend improvements.
  • Provide regular reports on outreach progress and program impact.
  • Adhere to all state and federal standards of care and reporting requirements
  • Assist in data collection and outcomes reporting, as needed, to support evaluation of outreach efforts and organizational goals.
  • Maintain client and program records, accurately and appropriately, in accordance with agency, regulatory, and funder requirements and timelines
  • Attend all relevant case conferences and staff meetings, including MDT
  • Provide onboarding and weekly supervision to outreach staff, including but not limited to: Promotores and Outreach Workers
  • Attend bi-weekly supervision
  • Other duties as required
