About The Position

This is a position exclusive to the Children’s Community Support Collaborative. Outreach Coordinators provide and oversee outreach services which include community-based individual and family therapeutic services, crisis intervention, and case management. They also are a member of CCSC’s Management team.

Requirements

  • High school diploma or GED
  • 2 years childcare and/or mentorship experience with a similar population.
  • Supervisory experience preferred
  • May be required to provide behavioral support
  • Required to carry a mobile device
  • Required to operate a vehicle (personal or agency-owned) and transport clients
  • Required to have a valid driver's license and a satisfactory driving record
  • Must be able to work with people of different backgrounds, languages and ethnicities
  • Frequent walking, standing, climbing stairs, and traveling to client home communities
  • Computer literacy, including Microsoft Office with the ability to learn new software applications

Responsibilities

  • Maintain a caseload of approximately 3–4 clients. Provide community-based counseling and therapeutic support to clients, support with case management tasks, and act as an advocate for clients.
  • Complete all required documentation for assigned cases.
  • Continuously communicate with internal and external collaterals and work with an interdisciplinary team. This may include extensive phone/electronic contact and scheduling and attending meetings.
  • Provide direct supervision to approximately 2–4 Outreach Department employees. Support and monitor performance, professional development, and accountability.
  • Facilitate regular Outreach Team meetings to ensure team coordination and connection.
  • Participate in Management Team meetings and contribute to organizational decision-making.
  • Serve in the Administrator-on-Call (AOC) rotation and respond to program needs during assigned coverage periods.
  • Participate in interviewing, hiring, and onboarding Outreach staff.
  • Participate in quality improvement initiatives and utilization review processes to ensure compliance with contract, licensing, and regulatory standards.
