About The Position

The role of the Outreach Coordinator is to assist with the promotion of Project Forward, the Agency and its programs to the community. Will work with existing agency personnel and a variety of outside agencies to provide education on the existence of healthy relationships workshops and other substance use/ mental health prevention services through the agency.

Requirements

  • High School Diploma or equivalent
  • One to four years of related human services experience required.
  • Knowledge of community resources/entities and mental health and recovery services.

Responsibilities

  • Act as liaison between the Agency and the communities it serves.
  • Represent the Agency in a professional and knowledgeable manner.
  • Assist in the promotion of the Agency through information and education about its services to the communities we serve.
  • Provide direct outreach to courts, attorneys, hospitals, probation, parole, DSS, CPS, DDP, PCPs, schools and various other community groups with direct referral back to the Agency and its programs.
  • Represent the Agency at various community events such as, but not limited to conferences, trainings, fairs, forums, etc.
  • Assist in the creation of flyers and promotional material with, and under the direction of, the director(s).
  • Document and report on contacts made in the community. Documentation should include name of contact, agency (if applicable), address and telephone number.
  • Assist Intake Specialist and Program Manager with contacts to individuals contacted during events/engagements and document on the appropriate tracker.
  • Engages with community members and schedules attendance at events, or presentations about program offerings (i.e., universities/colleges, libraries, medical agencies).
  • Schedules indoor and outdoor events for primarily Project Forward staff, as well as for other CNG events.
  • Organizes all items, flyers and give-a-ways for all events.
  • Manages/takes lead of staff who volunteer at events.
  • Monitors and Manages Give-a-way supplies, notifying management when to reorder.
  • Facilitate sharing of data with the individuals to whom it applies to facilitate partnered decision-making
  • Complete QI Training during onboarding and participate in QI refresher training as needed.
