About The Position

Coordinates, plans, implements and organizes the ERSEA (eligibility, recruitment, selection, enrollment and assignment) functions in addition to planning, organizing, and implementing the Family and Community Partnership service area of the South Bronx Early Head Start Program.

Requirements

  • Strong communication skills
  • Bachelor's Degree - Social work or related degree/Health related degree
  • Experience working with high risk families – 2- 3 years experience
  • Experience conducting outreach and doing presentation to patients and community based agencies – 1-2 years
  • MS Office Suite required (Excel, Word, PowerPoint)

Responsibilities

  • Establish and maintain positive relationships with patients, visitors, and other employees. Interacts professionally, courteously, and appropriately with patients, visitors and other employees. Behaves in a manner consistent with maintaining and furthering a positive public perception of BronxCare Health System and its employees.
  • Contributes to and participates in the Performance/Quality Improvement activities of the assigned department. Contribution and participation includes data collection, analysis, implementation of and compliance with risk management and claims activities, support of and participation in Continuous Quality Improvement (CQI) teams, consistent adherence to the specific rules and regulations of the BronxCare Health System (a) Safety and Security Policies, (b) Risk Management: Incident and Occurrence Reporting, (c) Infection Control Policies and Procedures and (d) Patient and Customer Service.
  • Identify and work from list provided by Manager locations for recruiting and engaging youth and youth adults who are at risk for HIV and STIs not connected to care to participate in Youth Access Program.
  • Working from schedule provided by Manager, attend events for recruitment and engagement of youth and young adults at risk for HIV and STIs.
  • Engage and assess youth and young adults for the program on the mobile unit.
  • Perform HIV and STI rapid tests on youth and young adults who are at risk for HIV and STIs.
  • Connect those patients to care who require follow-up testing, counseling, medication, and engagement in the program.
  • Market and distribute information about the program at schools, community organizations, other hospital and health centers, events, etc.
  • Receive referrals for the Youth Access Program and assess the at-risk youth and young adults for services.
  • Complete Program documentation and submit to manager for approval and then entry into AIRS tracking system.
  • Participates in staff meetings- both operational and programmatic- as required with manager and any other Departmental meetings as related to the Program.
