InterCare is searching for a full-time Community Health Worker to join our Outreach Team for the Migrant Season! Work Schedule: Hours are Sunday - Saturday; variable with evenings and weekends as necessary. This position is schedule to end on 11.01.2025. Minimum Qualifications Possesses a high school diploma or equivalent.. Primary Accountability Responsible for helping patients and their families to navigate and access community services, other resources and adopt healthy behaviors. The Community Health Worker builds individual and community capacity by increasing health knowledge and self-sufficiency through a range of activities such as outreach, community education, informal counseling, social support and advocacy.
