Requirements

  • Bachelor’s degree and one year of experience in a human services environment, or an equivalent combination of post-secondary education and relevant experience.
  • Must be at least 21 years of age, is required.
  • Willingness to submit to record checks to detect criminal convictions of behavior harmful towards children, spousal abuse, distributing drugs or sex crimes.
  • Requires a valid auto-insurance, a valid driver’s license or the ability to obtain one with approved driver status.

Responsibilities

  • The Outreach Case Manager works within the Arise Youth Center, which serves as an alternative to youth detention.
  • The Center provides services for young people who are involved in the juvenile justice system, are runaway or homeless, or are at risk of becoming so.
  • In this role, the Case Manager partners closely with youth and their families to offer support, coordinate services, and help clients work toward their personal goals.
  • As part of the LSS team, the Case Manager helps develop individualized case plans, conducts ongoing assessments, and connects youth with appropriate community resources.
  • This position is essential in identifying the needs of youth and their families, supporting their growth, and fostering positive change in their lives.
  • Success in this role requires strong communication and organizational skills, along with a deep commitment to helping others.
