About The Position

The Outreach and Community Relations Specialist is responsible for driving census growth across skilled nursing and senior living community levels, including assisted living, memory care, and independent living. This role focuses on developing strong referral networks with hospitals, skilled nursing facilities, physicians and other healthcare partners while identifying nd converting opportunities to place residents at the most appropriate level of care within the community. The specialist will use strategic marketing, relationship building and consultative sales approaches to increase admissions and enhance the organizations visibility within the healthcare and senior care markets. The Outreach & Marketing Specialist has a direct reporting responsibility to the Chief Operating Officer.

Responsibilities

  • Build and sustain strong relationships with hospitals discharge partners, case managers, social workers, physicians, SNF leadership teams, and other referral sources to drive admissions and increase census. Follows the strategic marketing plan components.
  • Conduct outreach visits to hospitals, skilled nursing facilities, and healthcare providers to promote the community’s services and care levels.
  • Identify opportunities to “sell” or match prospective residents to the most appropriate community level (e.g. assisted living, memory care, independent living) based on needs and qualifications.
  • Develop and executive targeted outreach and marketing campaigns to increase awareness of the community’s clinical capabilities, respite options, and long-term care services
  • Serve as a liaison between referral partners, prospective residents and internal admissions or sales teams to ensure a smooth and timely transition into care.
  • Monitor census trends, referral patterns, and competitor offerings to identify growth opportunities and adjust strategies accordingly.
  • Represent the organization at community events, professional conferences, health fairs and networking opportunities to expand referral relationships and strengthen brand presence.
  • Maintain accurate tracking of outreach activities, leads, referrals, and conversions to support ongoing performance analysis and continuous improvement.
  • Follows the strategic marketing plan components.
  • Must have a sense of urgency and be able to handle many activities and customers at one time.

  • Makes referrals to the appropriate supervisor of residents' needs.

  • Demonstrates flexibility with a changing workload/caseload, sets priorities accordingly and carries out job assignments in a self-directed manner.

  • Works closely with Residential Living Sales Manager in receiving coaching and guidance with the sales and marketing processes.

  • Works as a team player within the department and with other departments in the facility.
  • Assists co-workers when own work is thoroughly completed and/or is asked by the appropriate supervisor to do so.
  • Works in accordance with established safety guidelines with emphasis on the use of proper body mechanics and safe work practices; wears appropriate safety gear.
  • Maintains attendance as outlined in O’Connor Woods policy.
  • Attends and participates in appropriate in-service and department meetings.
  • Promptly reports noticeable environmental or equipment repair needs to the maintenance department.
  • Follows prescribed dress code. Maintains a professional image to the public and residents.
  • Possesses the natural ability to sell and the skill to implement strategic plans and set priorities. This position requires the ability to relate well to older adults and their family members and have sensitivity to the special needs and concerns of older people. Public Speaking and making effective presentations is required. Must have the ability to communicate verbally and in writing. Administrative ability to organize and prioritize various aspects of operations in coordination with other departments within the community and corporate structure.
  • Ability to work in a changing environment with frequent public contact and frequent interruptions and to maintain effective and positive working relationships with public contacts, as well as all levels of management throughout the O’Connor Woods structure. Must be able to adapt quickly to changing circumstances.
