About The Position

The Community Outreach Specialist will assist with the development and implementation of a community outreach and marketing strategy that will identify eligible patients targeting populations including, but not limited to, HIV/AIDS, Homeless, Migrant Seasonal Farm Worker, students in Pueblo County, and those eligible for Medicaid enrollment. Proven ability to work collaboratively and productively with clinicians, administrators, patients, and other community entities. Ability to grow and facilitate ongoing partnerships/relationships. The Community Outreach Specialist will be responsible for scheduling, planning, attending, educating, and coordinating the PCHC staffing of the community scheduled events to ensure appropriate coverage, messaging, and professionalism.

Requirements

  • Bachelor’s degree required.
  • One to three years relevant experience in community outreach or marketing required (health/human services preferred).
  • An equivalent combination of education and experience may be considered.
  • Work experience demonstrating strong ability in the following areas: leadership, advocacy, communication skills (both written and verbal), resource research, creative problem solving, and sensitivity/responsiveness to varying cultural characteristics and beliefs.
  • Valid driver’s license and auto insurance as required by law.
  • Frequent travel is required.
  • Ability to demonstrates intermediate to advanced conversational proficiency in English and the target language in customer service settings (will be required to pass language skills assessment test).
  • Able to work a flexible schedule (including occasional evenings or weekends) and have reliable transportation.
  • Ability to read and interpret documents and ability to compose information/instruction in a written form and write routine reports and correspondence.
  • Ability to work independently.
  • Ability to serve as an advocate for patients regarding specialty medical, oral health, mental health and substance abuse counseling referrals, appointments, prescription regimens, transportation, financial, social services, pharmaceutical companies, and support groups required.
  • Ability to prioritize multiple job responsibilities and exercise sound professional judgment.
  • Ability to enthusiastically communicate with enrollees, employees, community partners.
  • Strong computer skills with proficiencies in MS Office and internet-based applications.

Responsibilities

  • Ensure the function and activities of this department to embrace the philosophy, mission, values, and Communicate with Heart service model supported by the Board of Directors of Pueblo Community Health Center, Inc.
  • Adhere to the guidelines and procedures of Pueblo Community Health Center, Inc.
  • Performs non- clinical tasks that are part of his or her regular duties in a language other than English.
  • Provides language assistance in the place of another staff member when requested for customer service-related encounters (non-clinical) where understanding of healthcare/medical terminology/concepts is not required.
  • Provides language assistance only within the scope of the employee’s professional qualifications while demonstrating a high level of confidentiality.
  • Facilitate registration for the homeless, MSFW Program, persons living with HIV/AIDS, school-based wellness centers, and Medicaid eligible.
  • Plan, monitor, coordinate, and provide all outreach activities for the homeless, MSFW, HIV/AIDS, and school-based programs, including all affiliated projects (i.e. migrant Head Start, special clinics, Worlds AIDS Day, Pride Fest etc.)
  • Establish and grow relationships with referral sources
  • Educate the community about services provided by Pueblo Community Health Center
  • Seek and attend community events to generate enrollment of target population
  • Strategically collaborate with community partners
  • Provide person-to-person outreach to students/parents/staff in schools, the homeless, farm worker individuals/families, farmers, and HIV/AIDS population/prevention programs in the community.
  • Collaborate with various local organizations to build awareness of coverage options, spur enrollment, and build referral linkages.
  • Maintain open lines of communication and effective working relationships with other PCHC departments and agencies.
  • Assist in developing and modifying policies for special population programs.
  • Attend required meetings.
  • Represent PCHC on assigned community task groups and committees.
  • Attend and successfully complete all required training programs and participate in ongoing conference calls, webinars, and other professional development opportunities.
  • Establish and maintain effective working relationships with employees, the public, community-based organizations, and other agencies.
  • Work with the Communications Director to assure culturally appropriate messaging and promotion of outreach events
  • Manage and inventory the outreach materials, giveaway items, and preparation of materials for outreach events
  • Perform other related duties as assigned.
