Outpatient/Specialty Pharmacist Clinical fills prescriptions including those for specialty medications, monitor patient medication therapies, provide drug information, and supervises direct support personnel. This pharmacist performs patient outreach, schedule follow-up calls, document medication management, adverse effects, medication compliance and adherence as is required by accreditation agencies and supported by the patient management platform, EPIC Compass Rose. This Pharmacist may also support the benefits investigation team in seeking prior authorization for select high-cost medications where appropriate, in order to optimize Outpatient/Specialty Pharmacy's efficiency as well as enhance patient care and satisfaction, when needed.

Requirements

  • Education - Graduate of an ACPE-accredited school of pharmacy with either a BS pharmacy or PharmD required.
  • Experience - Hospital pharmacy experience is preferred. Retail pharmacy experience preferred, but not required. EPIC Willow Ambulatory experience is preferred, but not required.
  • Licensure - Currently licensed in the State of New York. Must be a certified immunizer in New York.

Responsibilities

  • Prepares and dispenses prescription medications and OTC products.
  • Communicates with providers to ensure safety and accuracy.
  • Provides excellent customer service.
  • Delivers consultative services to patients on proper medication use.
  • Assists in supervising and training of pharmacy support staff.
  • Complies with all New York State and Federal regulations.
  • Optimally manages pharmacy inventory and demonstrates understanding of storage and handling requirements that select medications may require.
  • Ensures proper reimbursement from third party payers.
  • Supports the technical staff as they conduct patient insurance, benefits and financial investigation and verification, and ensures it is directly communicated with patients.
  • Demonstrates an understanding of the prior authorization and insurance approval process that certain medications require and alerts ambulatory pharmacists of denials or nonresponses, where appropriate.
  • Facilitates communication and cooperation between all individuals involved in the medication management process in order to assure the accurate, timely and uninterrupted provision of pharmaceutical care.
  • For specialty pharmaceutical dispensing, provides education throughout the patient's journey on dosage, potential drug interactions, training on safe handling and administration, importance of adherence, as well as documents adverse drug reactions, failure to receive drugs, and treatment failures as required by the specialty pharmacy program.
  • Expedites access and adherence to care by assisting with calls to patients, documenting patient responses and providing ongoing consultation as is required to improve patient's clinical outcomes and meet standard accreditation performance.
  • Provides effective follow up with healthcare providers and caregivers (physicians, nurses, etc.) that supports continuity of pharmaceutical care and improved patient outcomes.
  • Maintains proficiency in and actively engages in operational pharmacy activities including specialty pharmacy areas as appropriate for job assignment
  • Reviews and resolves documented patient complaints with the Pharmacy Manager when necessary.
  • Supports the pharmacy management team in developing marketing materials and communication strategies to increase patient recruitment and prescription capture of high cost take-home medications.
  • Emulates mentorship skills and participates in experiential education for students as well as staff.
  • Is flexible in assuming other appropriate responsibilities not noted above.
