About The Position

Hamilton Diabetes and Endocrinology Center is a specialty education and physician practice focused on diabetes and endocrinology operated by Hamilton Medical Center (HMC). With the support and direction of the Program Director, the Nurse or Dietitian is responsible for patient education on diabetes, diet and nutrition. The Nurse or Dietitian will translate business knowledge into behaviors that produce high quality, cost-effective outcomes. This individual is able to apply program evaluation and accepted clinical research findings to secure clinical and program outcomes. He/she will build strong customer relationships, actively explore customer needs, and pursue creative solutions to satisfy those needs and monitor results on a continual basis. Ability to perform nursing services and set standards to assure professional, skilled program of care and treatment to support evidence-based standards of care. Nurse or Dietitian must possess exceptional communication skills and ability to accurately communicate education and instruction to an individual or group of individuals, must be comfortable with public speaking. Nurse or Dietitian is a self-motivated individual who can work independently, responsible for reviewing medical records, assessing patient needs, making recommendations to providers and educating the patient and/or family. Nurse or Dietitian is responsible for assisting practice manager and education coordinator with diabetes quality measures, educating staff as assigned, participate in community education activities, updating education materials, trouble-shooting disease specific equipment, participate in quality improvement initiatives and attend department meetings.

Requirements

  • Graduate of an approved program in dietetics or graduate of an accredited Diploma, Associate of Science in Nursing
  • Current unrestricted professional registration required
  • Current Georgia Registered Dietitian license required or current RN License in the State of Georgia
  • Current CPR Certification
  • The ability to communicate effectively with patients and professionals in diverse disciplines

Nice To Haves

  • Certified Diabetes Educator (CDE exam preferred after minimum number of hours obtained for exam)
  • Experience with diabetes patients preferred but not required
