About The Position

WestCoast Children’s Clinic (WCC) is a non-profit community psychology clinic in Oakland, California, providing mental health services to Bay Area children, youth, and families. WCC is client-centered, trauma-informed, collaborative, and committed to justice and equity. The Outpatient Case Manager works within dynamic teams including youth, caregivers, mental health providers, intensive care coordinators, social workers, teachers, and other supportive figures. This role involves providing a diversity of care, including intensive skill building, advocacy, linking to and coordinating various community resources such as tutoring, vocational programs, mentoring, shelter/housing, medical and mental health services, legal aid, education, and access to benefits. The Outpatient Case Manager also provides Clinical Case Management, Individual Rehabilitation or Intensive Home Based Services (IHBS) focusing on skill building, Enhanced Care Management (ECM) to connect youth and families to healthcare, and stabilization care for foster youth. The Outpatient Services program specifically addresses the needs of vulnerable and high-risk children, adolescents, and families, particularly those in the Alameda County foster care system, offering services at the Fruitvale clinic, via tele-health, and in community settings. An essential part of the position is timely completion of progress notes and other clinical documentation.

Requirements

  • At least two years of experience working with youth and families and/or providing case management, rehabilitation services, or intensive home-based services.
  • Passion for working with foster youth, families, and young adults who have complex traumatic histories.
  • High motivation to work from a team-based approach.
  • Values humility, compassion, self-care, and ongoing personal growth.
  • Flexible schedule with ability to work evenings and occasional weekends as needed.
  • Ability to travel by car to meet with clients, generally within a 90-mile radius of WCC's Oakland office.
  • Possess a clean CA DMV record.
  • Maintain current vehicle insurance when driving for WCC.

Responsibilities

  • Provide Clinical Case Management, including short-term and long-term intensive linkage and follow-up with community resources to improve mental health symptoms.
  • Provide Individual Rehabilitation or Intensive Home Based Services (IHBS) focused on skill building to improve life functioning, such as guidance on applying to jobs/schools, building self-care and hygiene skills, and strengthening social, coping, and other behavioral skills.
  • Provide Enhanced Care Management (ECM) to connect youth and families to healthcare and other services.
  • Provide stabilization care to foster youth in a milieu environment while they await and prepare for transition to foster placements.
  • Write progress notes for each meeting and complete other clinical documentation in a timely manner.
  • Demonstrate cultural humility, including awareness of the impacts of power, privilege, race, and SOGIE on self, clients, and others.
  • Manage time and schedule effectively, prioritize tasks, and complete timely documentation.
  • Manage a caseload, ensuring 60% (24 hours per week) are dedicated to providing services to youth and families.
  • Uphold ethical and legal responsibilities in clinical care.
  • Complete high-quality documentation, link youth to resources, support skill building, assess needs and strengths, and provide crisis intervention and de-escalation.
  • Work collaboratively as part of a multidisciplinary treatment team to support youth and families in developing and progressing toward their goals.
  • Provide services in a variety of community settings such as homes, schools, parks, shelters, and other agencies.
  • Build healthy relationships to help youth navigate complex systems and understand how trauma can impact youth and team dynamics, being open to Restorative Justice Practices.
  • Use supervision effectively, be self-reflective and accountable to growth areas, and participate in ongoing training provided by WCC.
  • Acquire knowledge of or ability to learn Medi-Cal reimbursement procedures and billing, and complete daily progress notes and other clinical documentation compliant with WCC policies and EPSDT Medi-Cal billing requirements.
