The organization is seeking an Outpatient Care Coordinator to support its psychiatric outpatient team, with a primary focus on medication coordination. This role involves managing refill requests, resolving prescription issues, and ensuring timely follow-up with providers and pharmacies. The position also works in coordination with front office staff to support scheduling, patient communication, and overall clinic flow, while ensuring documentation is accurate and completed on time.
