About The Position

The Orthopedic Services Navigator is a healthcare professional experienced in the management of patients with orthopedic surgical procedures. The Orthopedic Services Navigator provides guidance of unique orthopedic surgical patients through the continuum of care and development of educational programs for patients, family and staff as well as act as a liaison between AnMed Health and community-wide resources to optimize outcomes.

Requirements

  • BSN required, Master's Degree preferred
  • Current RN licensure for State of South Carolina required.
  • Must have a minimum of two years of work in a clinical setting and preferably two years of experience in orthopedics.
  • Current SCRN through ANCC or eligible within 24 months; possess a working knowledge of the process of services and the continuum of care; demonstrates problem-solving and/or critical thinking skills to reach best solutions based on factual information; serves as a role model to nursing staff in the delivery of expert patient care for their patient population.

Responsibilities

  • Contributes to the ongoing education and professional development of nursing staff and clinical teams for optimization of outcomes specifically with Total Joint Academy patients and Fracture Care Management.
  • Makes contact with all patients/family members, explaining the role of the Orthopedic Navigator. Serves as patient/family advocate, incorporating respect for and acknowledgment of cultural diversity and individual needs. Provides formal education to patient/family with appropriate documentation and follow-up.
  • Assists in the coordination of additional tests ordered at time of consult, providing information to patients, family and staff as needed. Assists in acquisition of needed/appropriate resources for the patient to reduce LOS. Provides and coordinates appropriate discharge planning and follow-up. Practices appropriate documentation for care provided.
  • Collects data related to patient outcomes for Ortho Scorecard and presents to Ortho Service Dept for Quality Review. Collects and submit timely data reports to include Stroke Care Measures and Stroke Get with the Guidelines.
  • Investigates patient care solutions to specific patient problems by reviewing current literature and collaboration with AVP of Orthopedic Services and Ortho Total Joint Medical Director/Ortho Service Chief.
  • Other duties as assigned
