About The Position

Heartland Alliance Health’s (HAH) mission is to transform healthcare for the most vulnerable – particularly people experiencing homelessness, mental illness or addictions, or struggling with multiple chronic illnesses – improving health for all and the well-being of our community. The Oral Health Educator will implement outreach activities to increase access to dental services, provide oral health education to targeted populations, and offer case management to dental patients to encourage completion of treatment plans. The role also includes training and coordinating Oral Health Promoters to deliver community-based oral health education. Individual accountabilities and work volume will be established annually through Success Objectives.

Requirements

  • High school diploma or equivalent required.
  • Minimum of one year of community health experience, related social service experience, or equivalent combination of education and experience.
  • Ability to work effectively in the community with the target population.
  • Excellent interpersonal and oral communication skills.
  • Proficiency in Microsoft Office, Internet Explorer, Outlook/Exchange, Windows operating systems, and other software routinely used by Heartland Alliance Health.

  • Bilingual in English and Spanish may be required.

Responsibilities

  • Learn the oral health outreach curriculum and train Oral Health Workers to perform targeted outreach activities.
  • Provide oral health education to existing and new patients.
  • Manage patient care follow-up via phone calls and other outreach to promote treatment plan completion and support other outcomes; assist with reporting as needed.
  • Conduct outreach to community partners (e.g., WIC offices, Head Start/Early Head Start programs, health fairs, CPS schools) to arrange and deliver oral health education to at-risk populations.
  • Participate in community fairs, create and distribute flyers and newsletters, manage social media efforts related to oral health, answer questions, and explain program services.
  • Promote dental services at community events and outreach activities to increase utilization of the dental program.
  • Deliver oral health education with cultural competence and sensitivity.
  • Train, collaborate with, and coordinate the scheduling and activities of Oral Health Workers.
  • Perform other job-related duties as assigned.
