About The Position

The Family Support Worker serves under the Chronic Disease Program facilitating health and wellness classes and presentations in the community. This position is also responsible for conducting outreach promoting our chronic disease prevention and management programs to community partners and medical providers. The Family Support Worker position will be expected to engage with internal and external partners to ensure they are meeting the needs and expectations of the community and the agency.

Requirements

  • Ability to build and maintain partnerships with community organizations and stakeholders.
  • Knowledge of the principles and practices of health promotion and public health education.
  • Knowledge of the principles and practices of community-based health promotion planning.
  • Ability to present accurate, science-based information to a variety of audiences.
  • Ability to develop, plan, implement programs.
  • Ability to work with youth and adults.
  • Ability to plan, prioritize and organize work assignments.
  • Skilled in oral communications.
  • Ability to prepare reports.
  • Ability to work independently, with minimal supervision.
  • A minimum of 6 months of experience in health education or outreach, including presentations, workshops, or program promotion in academic, clinical, or community settings.
  • A minimum of 6 months of experience supporting individuals with chronic conditions like diabetes or obesity through education, coaching, or resource connection.
  • A minimum of 6 months of experience facilitating or assisting with group sessions focused on chronic disease prevention or healthy lifestyle promotion.
  • A valid driver’s license.

Nice To Haves

  • Trained as a Lifestyle Coach to teach the CDC’s Diabetes Prevention Program.
  • Bilingual in Spanish and English.
  • At least 6 months experience facilitating group education for young children.

Responsibilities

  • Facilitate the Get Into Fitness Today 12-week wellness program to groups in the community.
  • Facilitate the Get Into Fitness Together- Learning Emotional Activity Program for kids 5-9 years old.
  • Provide classroom/group setting education in various locations throughout Hillsborough County as assigned.
  • Ensure Community needs are considered in identifying strategies for delivering program.
  • Coordinate program activities with the Chronic Disease staff and other health department staff as needed.
  • Promote improved nutrition and increased physical activity in the community.
  • Outreach to community organizations to promote the Get Into Fitness Today program and to help increase self- and partner referrals into the program.
  • Develop program marketing material to use when conducting outreach.
  • Promote Get Into Fitness Today program to medical providers and encourage patient referrals as needed.
  • Market to varied population groups to increase inclusiveness and health equity for all.
  • Review program participant outcomes and assist participants in setting plans for reaching weight and wellness goals as related to the Get Into Fitness Today program.
  • Communicate with program participants and provide reminder calls/texts/emails as needed to encourage completion of the program.
