About The Position

Oncology Pharmacist will facilitate the delivery of safe, effective, and pharmacoeconomically rational medications to hematology/oncology patients. They will provide education, develop and implement guidelines, policies, and medication regimens, provide appropriate interventions, and be involved in professional development and performance improvement initiatives. They will be responsible for supervising and directing support personnel in addition to the safe and proper distribution and use of medications in hematology/oncology patients. In the absence of pharmacy management, the Oncology Pharmacist may be required to assume the essential responsibilities and perform the duties of the Pharmacist in Charge (PIC).

Requirements

  • Bachelor of Science in Pharmacy or Doctor of Pharmacy degree
  • Current Pharmacist’s license issued by the Oregon State Board of Pharmacy.
  • Basic Life Support certification with expectation of Advanced Cardiac Life Support certification within first year of hire.
  • Postgraduate residency with oncology focus or equivalent experience in Oncology Pharmacy required.

Nice To Haves

  • Board Certification in Oncology (BCOP) preferred.

Responsibilities

  • Accurately compound and dispense pharmaceuticals including Hazardous medications
  • Monitor drug therapy regimens for contraindications, interactions, allergies, and appropriateness of drug and dose
  • Aid in the creation and modification of hematology/oncology treatment plans and infusion therapy plans
  • Sustain the hospital formulary, minimizing nonformulary procurements, utilizing therapeutic interchanges, and promoting rational drug therapy selection
  • Work varying shifts and assume on-call duties as necessary
  • Work independently with minimal direct supervision.
  • Work cooperatively with providers and pharmacy staff.
  • Handle frequent interruptions, adapt to changes in workload, and work schedule.
  • Set priorities, make critical decisions, and respond quickly to emergency requests.
  • Exercise sound professional judgment and communicate effectively (orally and in writing).
  • Participate in the training of new pharmacists, pharmacy students, technicians, nurses, and physicians.
  • Support and participate in the goals of the organization’s quality improvement, risk management, and patient safety program including but not limited to National Patient Safety Goals, Core Measures, and HCAHPS medication related initiatives.
  • Recognize and adhere to the rights and responsibilities of patient confidentiality (HIPPA).
  • The employee supports the hospital mission, vision, values, policies, and procedures.
  • Participates in required education for DNV programs as applicable to position (reference program education curriculum).
  • Performs other related duties as assigned.
