About The Position
Our client, a value-based cancer care company, is seeking a compassionate remote Oncology Nurse Navigator to join their team! The Oncology Nurse Navigator will provide triage, support, and education to members during their cancer journey via phone, email, and video. This is a full-time (40 hours) position, and candidates must be available to work Saturdays.
Requirements
- OCN certification required.
- Active compact nursing license required.
- Recent oncology navigation or oncology case management experience required.
- Minimum of 5 years of nursing experience, including at least 2 years in direct patient care experience in outpatient infusion or navigation.
Responsibilities
- Establish trusting relationships with members and their care network.
- Support members throughout their cancer care journey, including screening, survivorship, and end-of-life care.
- Assist members with care coordination, symptom management, nutritional support, discharge planning, and provider referrals.
- Assist with urgent clinical escalations and provide clinical consult support.
