About The Position

The LPN is responsible for providing direct and indirect patient care in the Oncology department. Provides care that reflects initiative, flexibility, and responsibility indicative of professional expectations with minimal supervision. Determines priorities of care based on physical and psychosocial needs, as well as factors influencing patient flow through the system. Communicates with providers about changes in patient's status, symptomatology and results of diagnostic studies.

Requirements

  • Current IL LPN License Required
  • BLS Required
  • LPN minimum 2 years Preferred
  • Oncology Care minimum 2 years Preferred
  • Strong computer skills with the ability to navigate multiple EMR programs
  • Ability to multi-task, prioritize and work independently in a team environment
  • Excellent ability to thrive under pressure and adapt to changes in assignment
  • Excellent communication and customer service skills

Responsibilities

  • Performs accurate and complete assessment of patients; coordinates and ensures completion of interdisciplinary screenings and assessments; assesses and reassesses pain; assessments are age, culture and psychosocial appropriate.
  • Explains specialty procedures to patient/family; utilizes specialty equipment; assists physician(s) with procedures; utilizes other departments to provide interdisciplinary care as appropriate; utilizes pain management techniques.
  • Educates the patient/ family regarding treatment plan and follow-up instructions given per provider instructions including any education handouts.
  • Prepares and administers medications based on age / clinical condition of the patient per policy and nursing standards of care; manages supply of medications and contacts pharmacy as needed to ensure uninterrupted medication therapy.
  • Follows medical records documentation guidelines regarding timeliness of reviews and completeness of documentation; obtains and documents accurate and thorough patient medical history including pertinent history, pain assessment, current medications, identified health risk factors and other needed information.
  • Performs all aspects of the position in an environment that optimizes patient safety and reduces the likelihood of medical/healthcare errors; utilizes established hand-off communication protocols; maintains the security of the department.
  • Complies with all established safety procedures to ensure a safe environment for patients, visitors and staff.
  • Participates in performance improvement activities.
  • Performs other duties as assigned.
