Position Overview: The Oncology Clinic Nurse- Care Coordination & Navigation is a registered nurse role designed to support comprehensive, longitudinal oncology patient care across the continuum. This position functions in a hybrid model, with approximately one (1) week onsite and three (3) weeks remote, with scheduling flexibility based on clinic needs and patient care demands. This role focuses on care coordination, chronic care management, patient navigation, preventive oncology services, and clinical follow up, ensuring prompt, high quality, and patient centered oncology care. The nurse collaborates closely with providers, clinic nurses, ancillary services, primary care providers, and external referral partners.

Requirements

  • Current, unrestricted Registered Nurse (RN) license in North Dakota.
  • Active BLS and ACLS certifications (for onsite work)
  • ONS Chemotherapy/Immunotherapy Provider Certification
  • Maximum one year of oncology nursing experience (clinical/care coordination experience preferred).
  • Ability to communicate effectively in English, both verbally and in writing. Other languages are desirable.
  • Basic computer knowledge

Responsibilities

  • Clinic Visit Follow Up Support Help clinic nurses and providers by reviewing charts following oncology visit to ensure the following. All labs ordered, diagnostics, and referrals are placed. Outside scheduling is completed promptly Following up with oncologist is completed, Results are tracked, routed, and communicated to the patient appropriately. Close the loop on incomplete orders or missed follow- ups.
  • Clinical Care Coordination & Navigation Coordinate care for oncology patients across internal and external services, including: Diagnostic Testing Specialty referrals (surgery, radiation, ets.) Procedures Ancillary services Track and ensure completion of labs, imaging, referrals, and follow-up appointments after clinic visits. Proactively find acre gaps and intervene to prevent delays in diagnosis, treatment, or follow-up. Serve as a clinical liaison between oncology providers, clinic nursing staff, outside facilities, and primary care providers (PCPs).
  • Chronic Care Management Manage assigned chronic care management (CCM) oncology patients in collaboration with CCM RN. Perform weekly and/or monthly patient outreach via phone or telehealth, based on patient needs and care plans. Monitor symptoms burden, treatment tolerance, adherence, and overall care needs. Document CCM activities accurately to support quality metrics and billing requirements (if applicable). Communicate with proper interdisciplinary team members as appropriate.
  • Long-Term & Pain Medication Monitoring Support long-term & pain management for oncology patients through: Regular weekly or monthly nurse calls (as instructed by physician) Symptom and pain assessment (using proper documentation in EMR) Medication adherence monitoring Identification of safety concerns or escalation needs Communicate changes or concerns to the oncology provider team promptly. Communicate changes back to patient and dose adjustments/lab monitoring, as necessary.
  • Germline & Prevention Service Line Navigation Serve as the primary nurse navigator for the Germline Prevention/Genetic Testing service line. Responsibilities: Intake and navigation of incoming patients Data collection and tracking Scheduling appointments Ordering germline/genetic testing per protocol and provider orders Follow-up test results with oncologists and patients, schedule follow up visits with oncologists/nurse practitioners. Patient education related to results and health maintenance implications. Coordination with PCPs or ordering providers for follow-up care and preventive planning. Maintain exact tracking and documentation for quality, compliance, and reporting purposes.
  • Collaboration & Communication Collaborate with: Oncology providers Clinic nurses Social work, nutrition, and other support services External referral partners Primary care providers
  • Documentation & Compliance Maintain thorough, exact, and prompt documentation in the electronic medical record (EMR). Adhere to all clinic policies, regulatory standards, and evidence-based oncology practice guidelines. Support quality improvement, audits, and data collection initiatives as needed.
  • Hybrid Work Expectations Work schedule includes approximately 1 week onsite and 3 weeks of remote, with flexibility based on: Patient needs Clinic workflow & staffing Program requirements Must be able to perform remote work duties independently while supporting productivity, responsiveness, and data security.
