About The Position

Requirements

• Active MN Registered Nurse (RN) license.
• Comfortable with travel and responding to calls across a wide metro area.
• Excellent at independent decision-making, prioritization, and triage.
• Compassionate communicator with patients, families, and team members.
• Tech-savvy enough to document and communicate from the field.
• Willing to participate in on-call rotations, including holidays and weekends.

Nice To Haves

• Minimum of 1 year of hospice experience strongly preferred.

Responsibilities

• Respond to patient needs during evenings, overnights, and weekends via in-person visits and phone triage.
• Deliver hands-on hospice care, including medication administration and comfort-focused nursing interventions.
• Guide families with empathy through symptom changes and end-of-life concerns.
• Maintain accurate, timely documentation using our hospice-specific HER.
• Coordinate with interdisciplinary team members to ensure seamless care delivery.
• Educate families on the dying process and what to expect.
• Provide reassurance, emotional presence, and expert communication during crises.
