About The Position

James River Hospice is a fast-growing, locally owned company focused on exceptional patient care and is seeking a compassionate Weekend Hospice On-Call RN in Lynchburg VA. This role involves phone triage, providing guidance to patients and families, and making in-person visits when needed to assess symptoms and offer support. You’ll work closely with the care team to ensure continuity of care and help patients maintain the best possible quality of life. Schedule: Weekends -Friday @ 5:00PM - Monday @ 8:00 AM

Requirements

Must be a Registered Nurse or Licensed Practical Nurse with a current license to practice nursing in the Commonwealth of Virginia
At least one year of hospice experience is required.
Graduate of an accredited nursing school.
Must have a valid driver's license and a reliable car available for use during work hours.

Responsibilities

Provide phone triage for hospice patients, assessing needs and determining appropriate care.
Conduct in-person visits as needed to assess, manage symptoms, and provide hands-on nursing care
Administer medications, pain management, and symptom relief interventions.
Offer emotional support and education to patients and families regarding end-of-life care.
Communicate and collaborate with the interdisciplinary team to ensure continuity of care.
Document assessments, interventions, and patient status accurately and timely.
Obtain and receive physician orders as required
Respond promptly to urgent patient needs and crisis situations.
Ensure compliance with hospice policies, procedures, and regulatory requirements.
Other duties as assigned
