This role is for an Occupational Therapist (OT) to deliver in-home, individualized therapy to pediatric clients in Lakewood, CO, focusing on improving independence, mobility, and overall quality of life through skilled, client-centered care.

Requirements

A valid Occupational Therapist license in Colorado
Proven experience in home health or a related field
Excellent communication and interpersonal skills
Proficiency in electronic medical record documentation
A flexible and adaptive approach to meet the diverse needs of your clients

Responsibilities

Develop and implement personalized treatment plans
Conduct evaluations using standardized tools and clinical expertise
Provide therapy focused on ADLs, mobility, safety, and independence
Educate patients and caregivers on home programs and adaptive equipment
Maintain timely and accurate clinical documentation
Collaborate with a multidisciplinary care team to ensure coordinated support
