Requirements

  • Active, unrestricted Colorado Occupational Therapist license
  • Ability to practice independently in a home-based setting
  • Strong clinical judgment and time-management skills
  • Clear, professional communication style
  • Reliable vehicle, valid driver's license, and auto insurance

Nice To Haves

  • Home health experience preferred (training provided if new to home health)
  • A genuine passion for helping people remain independent at home

Responsibilities

  • Provide skilled, individualized occupational therapy services in patients' homes
  • Complete OT evaluations, reassessments, recertifications, and discharge visits as assigned
  • Develop functional, patient-centered treatment plans focused on safety, independence, and quality of life
  • Perform home safety assessments and recommend adaptive equipment and environmental modifications
  • Educate patients and caregivers on treatment strategies, adaptive techniques, and home programs
  • Collaborate with an interdisciplinary care team (RNs, PTs, caregivers, families)
  • Supervise and collaborate with a qualified COTA , when applicable and assigned
  • Complete documentation efficiently with full EMR training and support
  • Travel locally within Mesa County (mileage and drive time reimbursed)
