About The Position

Adaptive Home Health is building a higher-acuity, patient-centered, skilled home health model across Texas. Our ultimate mission is to dramatically improve patient access to home health care. The OT role is the front line of that mission: you restore function in the home, build safe routines, reduce avoidable decline, and help patients regain independence in ADLs/IADLs. We build technology to better support our field clinicians faster and more supported. Ultimately, our clinicians are the customers of our engineering team. Our charting product is designed to reduce documentation burden and keep visits moving, and our scheduling product (launching soon) will make it easier to coordinate visits, reduce last-minute changes, and protect clinician time. If you have strong clinical judgment and you like owning outcomes, this role gives you autonomy, meaningful patient impact, and a support team built to remove administrative friction.

Requirements

  • Active Texas OT license (in good standing)
  • Strong clinical judgment and comfort working independently in the field
  • Comfort escalating changes in condition to RN/MD and documenting rationale clearly
  • Strong communication and documentation
  • Reliable follow-through on orders, frequencies, and patient goals
  • Current driver’s license and comfort with daily travel across assigned territory
  • Able to carry/transport basic therapy equipment as needed
  • Competency in core home health OT scope (ADL/IADL training, home safety, adaptive equipment, caregiver education)

Nice To Haves

  • Prior home health experience
  • Homecare Homebase (HCHB) experience (or similar home health EMR)
  • Deeper experience in high-signal OT populations (neuro, CVA, fall risk, dementia/caregiver training)
  • Strong organization and time management

Responsibilities

  • Complete in-home OT evaluations and deliver skilled OT visits.
  • Build and execute a clear plan of care focused on ADLs/IADLs, safety/fall prevention, and functional independence.
  • Recommend and train on adaptive equipment and home modifications as needed.
  • Coordinate with nursing and the broader care team when condition or needs change.
  • Document promptly and thoroughly in Homecare Homebase (HCHB) — defensible, efficient, and consistent.
