About The Position

We are seeking a compassionate Occupational Therapist to join our hospice team and support patients and families during one of life’s most important journeys. Our hospice team is intentionally built to provide more personalized, attentive care. With a focus on smaller caseloads and strong team collaboration, we ensure both our patients and clinicians receive the support they deserve.
Requirements
- Licensed Occupational Therapist in Tennessee
- Strong clinical knowledge of anatomy and rehabilitation principles
- Compassionate, patient-centered approach
- Excellent communication and teamwork skills
- Ability to work independently in a home-based setting
Nice To Haves
- Experience in hospice, home health, geriatrics, or skilled nursing preferred
Responsibilities
- Evaluate patients’ functional abilities and needs in the home setting
- Develop individualized care plans focused on comfort, safety, and independence
- Provide therapeutic interventions to improve quality of life and maintain function
- Educate patients and caregivers on adaptive techniques, energy conservation, and home safety
- Recommend and train on assistive devices and environmental modifications
- Collaborate closely with the hospice interdisciplinary team (nursing, social work, chaplain, aides, physicians)
- Accurately document patient care and progress
