This Full-Time Occupational Therapist Liaison position at St. Peter's Hospital is for an Occupational Therapist seeking a different type of role. The position is a member of the Complex Care interdisciplinary team and is responsible for ensuring patients transition to the next best site of care. The role involves directly co-treating patients with primary therapists, providing supplemental therapies, and utilizing clinical expertise to educate and provide resources for patients transitioning from acute care to the least restrictive environment. The liaison will serve the acute care & complex care team, promoting a 'no place like home' approach, and ensuring timely progression to the next site of care through appropriate occupational therapy treatment. Key responsibilities include educating case/care management teams, social work/utilization management services, and the broader hospital system on functional status, DME options, and alternatives to inpatient rehabilitation to maximize community re-entry opportunities. The role also involves problem-solving and collaborating on complex SIOH needs within the hospital.
