About The Position

This Full-Time Occupational Therapist Liaison position at St. Peter's Hospital is a unique opportunity for an Occupational Therapist looking for a different type of role. As a member of the Complex Care interdisciplinary team, the liaison is responsible for ensuring patients transition to the next best site of care. The role involves directly co-treating patients with primary therapists, providing supplemental therapies, and utilizing clinical expertise to educate and provide resources to ensure patients remain in the least restrictive environment when transitioning from acute care. The liaison serves the acute care & complex care team, promoting a 'no place like home' approach, and ensures timely progression to the next site of care by implementing appropriate occupational therapy treatment for vulnerable patients. A key aspect is educating case/care management teams, social work/utilization management services, and the broader hospital system on functional status, DME options, and alternatives to inpatient rehabilitation to maximize community re-entry opportunities. The position also involves problem-solving and collaborating on complex SIOH needs within the hospital.

Requirements

  • Graduate of an approved Occupational Therapy program
  • Current NYS Occupational Therapist (OTR) license and registration
  • 1 year experience as Occupational Therapist required
  • Excellent verbal, written communication, and interpersonal skills.
  • Strong project management skills.
  • Ability to inspire and build confidence in others and to forge alliances and garner support.
  • Strong analytical and critical thinking skills, including the ability to resolve complex administrative, technical or human situations.
  • Anticipate and resolve a broad range of issues on multiple, sometimes unrelated, topics, to determine the need for process development and revision.
  • Familiarity with standard desktop and windows-based computer system, including email, e-learning, intranet and computer navigation.
  • Ability to use other software required to perform essential functions.
  • Ability to utilize multiple electronic medical record platforms.

Nice To Haves

  • 5 years in healthcare environment, including health plans, hospitals, ambulatory or post-acute care preferred.

Responsibilities
