About The Position

HealthWorks Medical, LLC is looking for a self-motivated and team-oriented Nurse Case Manager to join our HealthWorks Marine Medical Management team. Hours are typically Monday through Friday 8:00 am until 5:00 pm (with telework options). Some early morning hours and some late afternoon short-notice overtime is also a possibility. Job Description Report: Acts as patient advocate in delivery and coordination of patient care. Provides case management to a group of patients providing specific assessments according to protocols and procedures Able to manage a large number of patient’s cases with high energy and organization Collaborates with other health care providers and Medical Director regarding patient needs. Review and assess medical records to determine employee’s ability to perform job duties Respond to workplace injury or illness by triaging initial telehealth call and providing follow up care.

Requirements

  • Ability to work independently and make decisions
  • Ability to effectively communicate with all levels of management, staff and partnering companies
  • Ability to organize, perform and prioritize multiple functions and tasks
  • Ability to organize, attend and participate in staff development, training, and wellness programs
  • Strong technical skills and the ability to adapt to new healthcare technology systems.
  • Minimum Education- Associates Degree or; one or two years related experience and/or training; or equivalent combination of education and experience.
  • Registered Nurse with a current KY license or valid compact state license OR LPN with a current KY license or valid compact state license
  • BLS certification - American Heart Association (Healthcare Provider) or American Red Cross (Healthcare Provider) required, must be current.

Nice To Haves

  • 1+ Year of Case Management Experience (preferred)
  • Bachelor's (Preferred)
  • RN or LPN: 3 years (Preferred)
  • Kentucky RN or LPN license (Required)
