About The Position

At CVS Health, we’re building a world of health around every consumer and surrounding ourselves with dedicated colleagues who are passionate about transforming health care. As the nation’s leading health solutions company, we reach millions of Americans through our local presence, digital channels and more than 300,000 purpose-driven colleagues – caring for people where, when and how they choose in a way that is uniquely more connected, more convenient and more compassionate. And we do it all with heart, each and every day. Fellowship Program Description: Start your NP career in the future of healthcare! Oak Street Health’s NP Fellowship in Adult Primary Care is in formal collaboration with the University of Michigan School of Nursing. The program is set in the revolutionary value-based care model, which rewards healthcare companies for the quality of their care, not the quantity. This 12-month paid fellowship will provide all the necessary skills, support, and learning required to become a highly confident and competent clinician for older adults. During the program, fellows spend time building their own patient panel, learning from a multitude of preceptors, and engaging in didactic content that aims to build on, not repeat, graduate training. All fellows commit to working at Oak Street for at least a year following program completion, but we hope they will spend their careers at Oak Street improving the health of communities across the country. The program has two concurrent cohorts starting in September and March of every year. The annual salary for this role is $100,000. Why Oak Street Health? Oak Street Health is on a mission to “rebuild healthcare as it should be'', providing personalized primary care for older adults on Medicare, with the goal of keeping patients healthy and living life to the fullest. Our innovative care model is centered right in our patient’s communities and focused on the quality of care over the volume of services. We’re an organization on the move! With over 160 locations and an ambitious growth trajectory, Oak Street Health is attracting and cultivating team members who embody “Oaky” values and passion for our mission.
