In an individual, group or telehealth under the direction of the registered dietitian, provides general nutrition and wellness education within their scope of practice. Refers patients to the RD for nutrition problems that are beyond the scope of practice for this position. Provides follow-up through monitoring and evaluation to the RD's nutrition care plan. Motivates patients to make changes and set individual nutrition goals by using Client Centered Services (CCS) and Motivational Interviewing Skills. Assesses patient's need for other health services through GLBHC or make appropriate outside referrals. Documents all work performed and outcomes of nutrition interventions in medical records (EMR) in a timely manner as required by GLBHC policy. Collaborates and communicates with members of the integrated care team to advance nutrition related care. Participates in meetings and huddles as needed. Participates in Quality and Process Improvement (QI) efforts, as needed. Assesses the need for patient education materials and other forms and assists in the development or procurement of these materials. Acts as a resource to all health center staff by staying current with nutrition knowledge, standards and practices of care. Participates and supports in providing nutrition services to other GLBHC programs (ex: Weight Management, School Based) as needed.
