About The Position

The Nutrition Assessment Specialist is responsible for assessing and re-assessing the eligibility of individuals for Non-State House Care Program referrals for home-delivered meals and refers program participants for other Services as needed. This position will also provide administrative support to the team as needed.

Requirements

  • Outstanding collaboration and communication skills with a cooperative nature; Possesses a strong ability to relate well to both clients and colleagues;effective communication skills both orally and in writing;
  • Comfortable with technology to include working within documents and spreadsheets and experience using a variety of software tools for the purpose of computer and record-keeping
  • Ability to drive, and dependable transportation

Responsibilities

  • Conduct in-home visits and phone assessments to assess and re-assess eligibility of non-state Home Care Program referrals for home-delivered meals and refer consumers to additional services as needed.
  • Complete all assessments and re-assessments for consumers in a timely manner, as assigned.
  • Make appropriate referrals for other internal and external needed services.
  • Ensure careful and thorough documentation of all assessments and referrals.
  • Perform other related duties as assigned by the Nutrition Program Director
