About The Position

The Visit Facilitator works primarily in family practice primary care and is responsible for supporting in person clinic visits, including communicating with patients prior to their visit and helping guide patients through the care experience. The visit facilitator gathers pertinent medical information from the patient and documents the information into the patient's medical record. This role also helps to ensure the exam rooms are stocked, equipment and supplies are available and assists with tasks that support the effectiveness and efficiency of the team. Here's what you'll do when you join us: Demonstrate a professional and compassionate attitude towards patients, families, visitors, coworkers and other departmental representatives. Prepare patients for their appointments, including communicating with patients prior to their appointment to ensure medical records are collected and ready for the visit. Prepare exam rooms and stocks supplies. Be a vital piece in the patient and provider connection ensuring patients and providers have a great experience. Complete visit wrap-up with patients. Provide patients with after-visit summary and recommendations for follow-up.

Requirements

  • Must be 18 years of age or older.
  • Graduate of a Nursing Aide program, Emergency Medical Technical (EMT) program, and/or Health Unit Coordinator program. Certification and current listing on the Minnesota Nursing Assistant Registry preferred.
  • BLS Certification or ability to obtain within two months of hire and maintains certification.
  • Minimum of 1-year customer service experience required. Previous health care customer service experience preferred.
  • Proficient with technology (computer hardware and programs, and smart devices such as phones and tablets).
  • Great verbal communication skills.
  • Strong team player with the ability to work independently.
  • Ability to adhere to strict confidentiality.
  • Demonstrated ability to adhere to and follow Grand Itasca Clinic & Hospital Mission, Values, Vision and policies.
  • Maintains reliable and consistent attendance.
