Requirements

  • High School Diploma or equivalent (required)
  • HHA eXchange experience (preferable)
  • Fluency in Spanish, Chinese or Bengali is a huge plus
  • Prior experience in home care, healthcare scheduling, or care coordination preferred
  • Strong verbal and written communication skills
  • Excellent organizational skills with attention to detail
  • Ability to prioritize tasks and manage time effectively in a fast-paced environment
  • Professional, positive, and team-oriented attitude
  • Strong customer service skills with a patient-focused approach

Responsibilities

  • Coordinate and schedule Field Nurses based on patient care needs, active physician orders, and nurse availability
  • Maintain ongoing communication with nurses regarding assignments, visit confirmations, attendance, and schedule changes
  • Ensure all physician orders and care plans are active and accurately documented prior to scheduled visits
  • Manage nurse schedules using HHA eXchange, including monthly service calendars and real-time updates
  • Confirm completed visits and process time-sensitive field paperwork for weekly payroll submission
  • Monitor nurse attendance, missed visits, late visits, and call-outs; arrange coverage as needed
  • Serve as a liaison between patients, families, nurses, and physicians to support quality home care services
  • Answer incoming phone calls and communicate with patients, families, physicians, and staff in a professional and reassuring manner
  • Maintain accurate documentation while ensuring patient confidentiality and HIPAA compliance
  • Communicate staffing challenges, scheduling concerns, and difficult-to-staff cases with the Director and internal team
  • Participate in on-call rotation with other scheduling team members
