About the Position Nursing Assistant - Visit Facilitator Temporary Flex .9-1.0 FTE approx. 6 months (72-80 hours per pay period) Days and Evenings On-site at Grand Itasca Clinic & Hospital in Grand Rapids INTERNAL POSTING THROUGH: 3/2/2026 Job Summary: The Visit Facilitator works primarily in family practice primary care and is responsible for supporting in person clinic visits, including communicating with patients prior to their visit and helping guide patients through the care experience. The visit facilitator gathers pertinent medical information from the patient and documents the information into the patient's medical record. This role also helps to ensure the exam rooms are stocked, equipment and supplies are available and assists with tasks that support the effectiveness and efficiency of the team. Here's what you'll do when you join us: Demonstrate a professional and compassionate attitude towards patients, families, visitors, coworkers and other departmental representatives. Prepare patients for their appointments, including communicating with patients prior to their appointment to ensure medical records are collected and ready for the visit. Prepare exam rooms and stocks supplies. Be a vital piece in the patient and provider connection ensuring patients and providers have a great experience. Complete visit wrap-up with patients.
