In compliance with regulation, company policy, and excellent nursing practice, the LPN is responsible for supporting wellness functions that may include, but is not limited to: documentation on electronic health records, communication with residents, families, vendors, physicians, pharmacy and staff; performing and monitoring health metrics for wellness and supporting the care team in addressing resident care issues and concerns. This role also facilitates an enriching community environment by recording resident vital signs, supporting ADLs for residents and the resident care team, and managing medication orders and documentation. The LPN develops collaborative relationships with third-party service providers, educates residents and families on healthcare procedures, and is responsible for staff training related to resident care. They provide direct assistance with ADLs, manage medication supply, and may conduct quarterly medication audits. The position requires active participation in understanding resident interests to recognize changes in condition, maintaining stock supplies for the care department, and ensuring cleanliness. The LPN is expected to exercise good judgment, resolve resident concerns, participate in workplace safety, contribute to an inclusive environment, model flexibility, and maintain confidentiality.
