About The Position

A key component of this job is to support and promote the residents’ independence by establishing care goals; teaching the residents to understand their condition, medications, and self-care skills.

Requirements

Successful applicants must have strong written and verbal communication skills
Must be able to provide good customer service, both in person and by telephone
Must possess good organizational skills, have a high attention to detail, have the ability to handle multiple tasks, prioritize work assignments, and maintain confidentiality.
Must have at least 2 years of experience working with substance abuse, mental health and homeless population.
A bachelor's of science in nursing and current New York State RN License and Registration required.

Bi-lingual Spanish a plus.

Responsibilities

Assess the health needs of all recipients and make appropriate medical referrals as needed.
Participates in the development of residents Individual Support Plans (ISPs).
Maintains accurate, detailed reports and records of symptoms or changes in residents’ conditions.
Documents encounters with the residents on a progress note.
Interprets and evaluates diagnostic tests to identify and assess residents’ conditions and shares information with the team.
Consults and coordinates with residents’ healthcare providers to assess, plan, implement or evaluate residents’ care plans.
Monitors all aspects of residents’ care, including diet and physical activity.
Facilitates groups for the residents on topics such as health education, disease prevention and assists the residents and the service staff to develop health improvement programs.
Maintains nursing supplies inventory by checking stock to determine inventory level; anticipating needed supplies; placing and expediting orders for supplies.
Some evening and weekend work will be required.
Other duties as required or assigned by supervisor.
