About The Position

The Department of Public Health is seeking a highly motivated, detailed orientated individual to serve as a Health Facilities Surveillance Nurse in the Division of Assisted Living. This individual will conduct surveys of facilities, including state licensure and complaint surveys. The HFSN will be responsible for preparing written survey documentation. Under general supervision, surveys facilities to determine compliance with state licensure requirements and Department directives, program policies and procedures. Travels to conduct routine health and safety surveys to determine compliance with state and federal requirements. Interviews facility residents, staff, administrators, health care providers and other individuals. Observes patient care provided at facilities. Reviews medical records, policies, standard operating procedures and other facility documents related to licensure and/or regulatory compliance.

Requirements

Requires (a) graduation from an approved nursing education program resulting in an associate or a diploma degree in nursing and three (3) years of professional nursing experience or, (b) a bachelor's degree in nursing and two (2) years of professional nursing experience or, (c) a master's degree in nursing.
Requires licensure as a Registered Nurse in the State of Illinois.
Requires appropriate, valid driver's license.
Requires ability to travel in the performance of duties with overnight stays as appropriate.
Overtime is a condition of employment, and you may be requested or required/mandated to work overtime including scheduled, unscheduled, or last-minute overtime. This requires the ability to work evenings and weekends.
Must reside in JoDavies, Stephenson, Winnebago, Boone, Carroll, Ogle, Dekalb, Whiteside, Lee, Rock Island, Mercer, Henry, Bureau, Putnam, or LaSalle County. (May be assigned to facilities outside that area, if necessary)
Requires ability to meet all agency vaccine/health-related polices and guidance.
Certain provisions of the revolving door restrictions contained in 5 ILCS 430/5-45 apply to this position. As a result, the employee should be aware that if offered non-State employment during state employment or within one year immediately after ending State employment, the employee shall, prior to accepting any such non-State employment offer, notify the Office of the Executive Inspector General for the Agencies of the Illinois Governor (OEIG) or may be subject to a fine.

Nice To Haves

One (1) year experience in assisted living care setting.
Three (3) years professional nursing experience.
Two (2) years' experience participating in the survey process for completion of all survey functions including but not limited to annual recertification, licensure, follow-ups, complaints, and incident investigation surveys.
One (1) year experience preparing complex written & oral reports.
One (1) year work experience in Microsoft Word, processing, and spreadsheet applications (i.e., Microsoft Word, Microsoft Outlook, Excel PowerPoint, Smartsheet).
One (1) year experience exercising judgement, discretion and maintain confidentiality during the conduct of the survey process.
Two (2) years of experience establishing, maintaining, and improving cooperative working relationships with various stakeholders.

Responsibilities

Under general direction, surveys Assisted Living and Shared Housing Establishments to determine compliance with state licensure requirements, and Department directives, program policies and procedures.
Prepares written survey findings.
Performs monitoring functions at noncompliant facilities.
Completes necessary surveyor training.
Provides technical assistance to facilities in emergencies.
Participates in staff meetings and assist with orientation of newly employed health facilities surveillance nurses and other Department staff as directed.
Performs other duties as required or assigned within the scope of the responsibilities enumerated above.
