As a growing organization, we are seeking a dedicated Hospice Nurse Practitioner to join our team and make a meaningful impact on the lives of those we serve. Position Summary: The Hospice Nurse Practitioner (NP) plays a crucial role in the interdisciplinary team by delivering high-quality palliative and hospice care to patients facing life-limiting illnesses. The NP will conduct assessments, develop and implement individualized care plans, and provide symptom management to ensure patients experience the highest quality of life possible. This role also involves supporting families and caregivers with education and emotional support.

Requirements

  • Master’s or Doctorate degree in Nursing from an accredited institution.
  • Current Nurse Practitioner license in NV.
  • Minimum of 2 years of experience in hospice, palliative care, or related field.
  • Strong clinical skills in pain and symptom management.
  • Excellent communication and interpersonal skills, with a compassionate and patient-centered approach.
  • Ability to work independently and as part of an interdisciplinary team.
  • Current CPR certification.
  • Valid driver’s license and reliable transportation.

Nice To Haves

  • Certification as a Hospice and Palliative Care Nurse Practitioner (preferred).

Responsibilities

  • Conduct comprehensive assessments of patients, including physical, psychosocial, and spiritual needs.
  • Develop and implement individualized care plans in collaboration with the interdisciplinary team.
  • Provide symptom management and pain control, adjusting treatment plans as necessary to improve patient comfort and quality of life.
  • Prescribe medications and therapies within the scope of practice and in accordance with state and federal regulations.
  • Educate and support patients, families, and caregivers on the hospice process, symptom management, and what to expect during the end-of-life journey.
  • Collaborate with physicians, nurses, social workers, chaplains, and other members of the care team to coordinate and deliver comprehensive care.
  • Participate in case conferences, team meetings, and quality improvement initiatives to enhance patient care.
  • Maintain accurate and timely documentation in accordance with regulatory requirements and organizational policies.
  • Provide on-call coverage as needed to ensure continuous patient care.
