About The Position

We are seeking experienced Nurse Practitioners and Physician Assistants to join an integrated care team focused on improving outcomes for patients with chronic illness, including kidney disease. This hybrid role blends in-home visits with telehealth, allowing providers to deliver personalized, evidence-based care where it matters most. You'll work closely with a Registered Nurse and Social Worker to improve quality of life, slow disease progression, and reduce avoidable hospitalizations.

Requirements

• Active, unrestricted NP or PA license
• Board certification (NP: ANCC/AANP | PA: NCCPA)
• Active DEA license
• 2+ years of direct patient care experience
• Experience with annual wellness visits (home or virtual preferred)
• Ability to practice autonomously
• Valid driver's license, reliable transportation, and insurance

Nice To Haves

• Managed Care / IPA / Health Plan experience preferred

Responsibilities

• Conduct comprehensive assessments and annual wellness visits (home and virtual)
• Develop, implement, and adjust treatment plans
• Prescribe medications and order diagnostic testing
• Coordinate care with PCPs, specialists, and ancillary services
• Educate patients and families on disease management and preventive care
• Maintain accurate, timely clinical documentation
• Participate in integrated care team meetings
• Follow up and evaluate patient progress
