About The Position

We are seeking experienced Nurse Practitioners and Physician Assistants to join an integrated home-based care team focused on patients with chronic illness, including kidney disease. This hybrid role allows you to deliver personalized, evidence-based care through a mix of in-home visits and telehealth, while working alongside a Registered Nurse and Social Worker.

Requirements

• Active, unrestricted NP or PA license
• Board certified (NP: ANCC/AANP | PA: NCCPA)
• Active DEA license
• 2+ years of direct patient care experience
• Experience with annual wellness visits (home or virtual preferred)
• Ability to practice autonomously
• Valid driver's license, reliable transportation, and insurance

Nice To Haves

• Managed Care / IPA / Health Plan experience preferred

Responsibilities

• Conduct comprehensive assessments and annual wellness visits (home and virtual)
• Develop, implement, and adjust treatment plans
• Prescribe medications and order diagnostic testing
• Coordinate care with PCPs, specialists, and ancillary services
• Educate patients and families on disease management and preventive care
• Maintain timely, accurate clinical documentation
• Participate in integrated care team meetings
• Follow up and evaluate patient progress
