About The Position
We are seeking Nurse Practitioners or Physician Assistants to join a home-based care team serving patients with chronic and complex conditions, including kidney disease. Youll work alongside a Registered Nurse and Social Worker to deliver personalized, patient-centered care through a mix of in-home visits and telehealth.
Requirements
- Active NP or PA license and board certification
- Active DEA license
- 2+ years of direct patient care experience
- Ability to practice independently
- Valid driver's license and reliable transportation
Responsibilities
- Perform comprehensive assessments and annual wellness visits (home & virtual)
- Develop and manage individualized treatment plans
- Prescribe medications and order diagnostics
- Coordinate care with physicians, specialists, and care team members
- Educate patients and families on disease management and preventive care
- Maintain accurate, timely clinical documentation
