About The Position
WellBe Senior Medical is transforming healthcare by focusing on Value-Based Care delivered directly in patients' homes, moving away from profit-driven volume. This approach allows practitioners to practice medicine with autonomy, connection, and purpose. The role offers a $10,000 Sign-On Bonus and extensive support, including daily team check-ins, ongoing training, and 1:1 mentorship. Practitioners will focus on outcomes-based medicine, addressing social determinants of health, managing a dedicated patient panel with interdisciplinary team support, and exercising true autonomy to prioritize patient well-being.
Requirements
- Advanced Degree: Master’s Degree (NP)
- GA Credentials: Active, unencumbered NP license and Board Certification (ANCC or AANP).
- Reliable Mobility: Valid driver’s license and vehicle for local field-based travel.

Responsibilities
- Deliver Field-Based Care: Conduct comprehensive primary care visits and physical exams in the comfort of a patient’s home.
- Manage Chronic Disease: Create longitudinal care plans that move the needle for high-risk patients.
- Collaborate Across the Continuum: Partner with an interdisciplinary team to ensure seamless, safe care transitions.
- Prevent ER Visits: Treat acute needs in the home to keep seniors safe, healthy, and out of the hospital.
