Nurse Practitioner/Physician Assistant – Primary Care – St. James/Vienna We’re seeking a Nurse Practitioner or Physician Assistant who shares our C.A.R.E.S. values: Compassion, Accountability, Respect, Excellence, and Service. Practice Highlights: Established patient volume of ages 6 months and older 5:1 call; one night per week and weekend rotation Collaborative colleagues, including 3 physicians and 2 nurse practitioners Dedicated nurse with floating staff based on patient volume X-ray, lab, and VFC vaccine programs available on-site Core Responsibilities: Preventive Care: Routine exams, screenings, vaccinations, and lifestyle education to prevent illness. Acute Illness Management: Diagnosing and treating common conditions like infections or injuries, with meds, tests, or minor procedures. Chronic Disease Care: Monitoring and treating ongoing issues (e.g., diabetes, hypertension) via follow-ups and adjustments. Care Coordination: Referring to specialists and integrating feedback for holistic patient management. Patient Counseling: Educating on health topics, mental wellness, and building long-term relationships.
