Requirements
- Active, unrestricted APRN license in Kentucky
- Board certified; Medicare credentialed
- 6+ months SNF or long-term care experience
- Comfortable with EMR and team-based care
- Able to work onsite in Monticello, KY
Nice To Haves
- Experience with HEDIS or STAR measures
- Multi-site rounding experience
