Nurse Practitioner Outpatient Support Special Location
About The Position
Legacy has a mission to improve the wellness of patients, the community, and each other. The organization is recruiting an Outpatient Advanced Practitioner for its Canby Clinic, located at 1433 SE 1ST AVE., Canby, OR. The clinic's providers include physicians and advanced practitioners, supported by individually assigned MAs and care teams comprising panel coordinators, social workers, RN case managers, behavioral health clinicians, and a pharmacist. Legacy operates over 25 clinics across the Portland metro area and rural locations, serving diverse patient populations and contributing to the health of the Pacific Northwest.
Requirements
- Graduate of NP or PA program
- Minimum of 2-years’ experience
- Current Oregon State license
- Current BLS certification from the American Heart Association
Responsibilities
- Responsible for the patient throughout the course of treatment, including reviewing clinical history, providing the physical examination, making the diagnosis, and treating both chronic and episodic disorders
- Work in collaboration with the team and seek the advice of a team physician whenever a case falls outside the scope of expertise
- Maintain own panel of patients
- Share call coverage for the clinic
