Requirements

  • Advanced Nursing Degree, Master’s level from accredited nursing school.
  • Current unrestricted APRN license, Florida.
  • Must possess/be eligible for Medicare license as a participating provider.
  • DEA license required.
  • Must possess a valid driver’s license in the state of Florida, current automobile insurance, and reliable transportation.
  • Certification of completion of Alzheimer's Disease and Related Dementias Training through the Florida Department of Elder Affairs.
  • Active BLS for healthcare professionals from the American Heart Association or Red Cross.
  • Knowledge of the principles and practices of primary medical care and palliative medicine.
  • Consent to drug and/or alcohol testing after a conditional offer of employment and ongoing compliance with the Drug-Free Workplace Policy.
  • Submission through the Florida Care Provider Background Screening Clearinghouse for eligibility verification.

Nice To Haves

  • Specialty in Adult Health, Primary Care, and Geriatrics preferred.
  • Hospice or Palliative Care experience.
  • Certification in Hospice and Palliative nursing.
  • Medicaid eligibility (preferred for Medicare license).
  • Credentialing through ANCC as a NP.

Responsibilities

  • Administer quality primary care in a professional manner which supports the patient and families.
  • Provide documentation in the patient record that meets criteria for good medical practice, addresses legal mandates, and adequately supports billing codes.
  • Provide float coverage for other PACE centers as needed.
  • Provide on-call coverage for patient/families on a rotation basis.
  • Communicate successfully with other Hope Healthcare services employees, the public, and community physicians.
  • Assist the Medical Director in maintaining 24 hour, 7 day per week medical coverage for PACE patients in Hope Hospice facilities and services.
  • Participate in Hope PACE medical staff meetings and complete any mandatory continuing medical education activities.
  • Participate in Quality Assurance activities approved by the administration.
  • Follow federal and state laws regarding medication prescription.
  • Represent the company in a positive and professional manner at all times.
  • Complete all job responsibilities in a thorough and complete manner, utilizing good judgment.
  • Carry out duties and responsibilities in a manner that exemplifies excellent customer service.
  • Proactively seek the assistance of immediate supervisor when unsure of a directed task or any part of job responsibilities.
  • Assist with special projects, as requested.
  • Perform all other job duties as may be assigned.
