About The Position

The Nurse Practitioner provides primary care to participants. Performs physical and periodic assessments of participants develop and implement appropriate plans of care. Provides participant and family education.

Requirements

  • Master of Science of Nursing Degree
  • Graduate of accredited Nurse Practitioner Program.
  • Certified Registered Nurse Practitioner (NP) in Missouri.
  • Registered Nurse (RN) in Missouri.
  • 3 Years' clinical nursing practice in an acute or primary care setting.
  • Current driver's license, auto insurance, and dependable transportation.
  • Medical clearance for communicable diseases and up-to-date immunizations, before having direct participant contact.
  • 2 years’ experience as a Registered Nurse with extensive experience with the elderly.
  • 1 year experience as a Nurse Practitioner in a geriatric setting.
  • 1 year working with the elderly population.
  • Knowledge of medical equipment and instructions.
  • Knowledge of common safety hazards and precautions to establish and safe work environment.
  • Skills in applying and modifying the principles, methods, and techniques of professional nursing to provide ongoing care for participants.
  • Skilled in identifying problems and recommending solutions.
  • Broad knowledge of medical conditions and appropriate management of conditions.
  • Skilled in preparing and maintaining records, writing reports, and responding to correspondence.
  • Ability to react calmly and effectively in emergency situations.
  • Ability to work effectively within an interdisciplinary team model, interfacing and collaborating with a wide range of clinical and social services disciplines who work together to manage the PACE participants' care.
  • Effective oral and written communication skills.
  • Effective interpersonal Skills with ability to build relationships.
  • Effective organizational skills with attention to detail and time management skills.
  • Proficient with Microsoft Office Suite or related software.
  • Proficient with Electronic Medical Records.

Responsibilities

  • Performs initial physical assessment of prospective participants.
  • Appropriately identifies and documents problems in medical records.
  • Performs periodic assessments of participants.
  • Updates the plan of care, problem list and data sheet with appropriate documentation.
  • Evaluates participants' physical complaints and orders appropriate diagnostic and therapeutic procedures.
  • Synthesizes data to determine preliminary diagnosis and therapeutic plan utilizing principles of prevention and promotion of functioning.
  • Performs blood draws, EKGs and dressing changes, and any other nursing interventions as needed.
  • At the direction of the Medical Director, review medical outcomes/statistical and utilization data, including review of necessity of hospitalizations and use of specialists.
  • When problems are identified work with the Medical Director to establish best practices and with clinical staff and interdisciplinary team to make appropriate procedural and or operational changes to avoid unnecessary hospitalizations and improve the health of participants.
  • Completes and accurate documentation of all appropriate diagnostic codes for each participant.
  • Advises the Medical Director in ways and means to establish better accountability of PACE services to participants and referral sources.
  • Manages medical emergencies.
  • Oversee the care needs of participants in rehab facility, nursing homes and hospitals.
  • Maintains up-to-date and appropriate documentation in the participant's electronic medical record.
  • Effective team members at morning meetings, intake and assessment, family meetings, care conferences and any other meetings as necessary.
  • Participates in the interdisciplinary team meetings, communicating participant changes, collaborating on care planning decisions and coordination of 24-hour care delivery.
  • Participates in in-service training and staff meeting as required.
  • Performs other duties as assigned.
