Requirements

Current CA State issued Nurse Practitioner license
Current active DEA license
1+ year as a practicing Nurse Practitioner
5+ years a Registered Nurse
Experience in elderly care/geriatric setting

Responsibilities

Serve as the primary clinical resource for urgent participant needs.
Perform rapid assessments, physical exams, and order/interpret diagnostic tests for triage cases.
Formulate treatment plans, prescribe medications, and provide immediate interventions as needed.
Manage participant and staff inquiries, prioritizing care and routing appropriately.
Educate participants, caregivers, and staff on urgent care protocols and health management.
Collaborate with interdisciplinary team members to ensure continuity of care.
Document all triage encounters and interventions in compliance.
Provide leadership in managing medical emergencies within the center.
Other duties as assigned.
