A Triage RN receives and prioritizes all incoming calls in order to continue delivery of safe, holistic care to a population with multiple co-morbidities and advanced chronic illnesses. The Triage RN implements the nursing process during phone encounters to assess the patient and caregiver situations and provide appropriate support including education of the disease process to the patient and family. Triage team working during core business hours are facility-based positions. Requirements

Minimum of 2-3 year Med/Surg, acute care (hospital) and/or post-acute care nursing experience required
Able to work weekend, holidays
Able to demonstrate professional competencies in a home care setting
Able to efficiently utilize technology (cell phone, computer)
Current State RN License
Current CPR Certification

Nice To Haves

1 year of home care experience preferred
BSN preferred
ER/Triage experience a plus

Responsibilities

Receives and prioritizes all incoming calls

Provide appropriate support including education of the disease process to the patient and family
